YDAS: In-Person Access Information Form

What is this form?

This form is for people and organisations interested in having YDAS run or take part in an in-person event at a location of your choosing.

Why do we need this information?

As our facilitators are all amazing disabled young people, we need to know if the space is accessible for them.

Please include information on both accessible and inaccessable features of the venue. The more we know, the better we can work around it. If the building has an access key, we can use that too.

How to fill out this form?

Pleasee be aware that the form is a fair amount of work, and it might be new to you - that's okay! We are here to support you through this process, and help you practice your access and inclusion even before we start the session!

Reminders & Acknowledgements

Where possible, we encourage all attendees to complete a Rapid Antigen Test on the morning of the event. If there are any positives, immediately contact the Together Program Officer on 0472 635 607. *

I acknowledge this and will encourage staff members to do RATs where feasible, and notify the Together Program Officer if anyone tests positive.

Attendees cannot come in-person if they are unwell or have any COVID-19 or other flu-like symptoms, even if they test negative on a RAT. We can set up online attendance if required. *

I acknowledge this and will ensure attendees know they cannot come in-person if they have any symptoms, and that hybrid delivery can be set up if required.

We don’t provide any catering, but if you will have any food present, we request knowing what it is so we can ensure it doesn’t cause any allergies in our facilitators. *
