We are Youth Disability Advocacy Service (YDAS). We have a project called In and Out and you can read more about it here. This survey helps us understand how effective or useful the In and Out project is.

Your answers are anonymous. That means we do not ask for your name, and we will not know who said what. We will share these answers with the Department of Health that funded this project, and other staff within YDAS that work on the project. We will use your answers to evaluate the project and make improvements for this and future projects.

You can fill in this form if you:

Are a consumer of the mental health system in Victoria
Are a worker in any part of the mental health system in Victoria
Supported someone to use the mental health system in Victoria
Used the In and Out website
Used the Booklet, including the prototypes, digital or print and outline
Read the In and Out report or executive summary
or in any other way used the In and Out project to help take part in the mental health system

Privacy and consent

If you submit this form then you are giving us consent to store and use the information you gave.

Only YDAS staff will have access to this form, but we may use parts of this information in a de-identifed way to report to Government Departments and other organisations about the groups of people we work with. De-identified means we will not share any identifying details. We may report on our work in an aggregated way (for example, showing the percentage of participants in different groups). We will also use this information to provide accessible and inclusive spaces for participants.
